About this service

Some patients need care at home or in the community rather than at the surgery. The Nantwich and Rural Care Community Team works alongside us to provide that. Most referrals to community services go through your GP.

What it covers

  • District Nurses for housebound patients needing wound care, injections, or palliative support
  • Community Physiotherapists for rehabilitation after surgery or stroke
  • Occupational Therapists for daily-living equipment and home adaptations
  • Macmillan Nurses for cancer patients and their families
  • Continence service and tissue viability nurses

How to access this

Speak to a GP if you think you need community support. We'll make the appropriate referral. Existing patients of community services can call them directly using the number on their care plan.
