We're accepting new patients. You can register online using the NHS registration form, or pick up a paper form from reception.

How to register

1. Online

Fill in the NHS online registration form. You'll need your NHS number if you have it, but it's not essential.

2. In person

Call into the practice during opening hours and pick up a registration form from reception. Fill it in and hand it back to one of our receptionists.

What happens next

Once we've received your registration form:

  • Your details are sent to PCSE (Primary Care Support England) who will notify your previous GP.
  • We'll request your medical records. This usually takes a few weeks to transfer.
  • You may be invited for an NHS health check with one of our nurses.
  • You'll be allocated a named GP who's responsible for your overall care. If you have a preference, tell us and we'll try to accommodate it.
  • You can book appointments straight away. You don't need to wait for your records.
You don't need proof of address or immigration status to register. Everyone in England has the right to register with a GP. We follow NHS England guidance and won't ask for documents that aren't needed.

We can register patients who live within our practice area. This covers Nantwich town centre and the surrounding villages including Acton, Henhull, Stapeley, Willaston, Weston, and parts of Crewe Road.

If you live outside our boundary but work in Nantwich, you may still be able to register as an out-of-area patient. Contact us to discuss.

No. It helps if you have it, but we can find it for you. You can also look it up at nhs.uk/find-nhs-number.
Yes. Photo ID, proof of address, and immigration documents are not required. NHS England is clear on this. We'll never turn someone away for not having ID.
Every patient is allocated a named GP responsible for their overall care. You don't have to see that GP every time. You can see any doctor or nurse at the practice. Your named GP makes sure your care is coordinated, especially if you have ongoing health needs.
Yes. You can use the practice address if you don't have a fixed home. Everyone has the right to register with a GP regardless of their housing situation.
Electronic records usually transfer within a few days. Paper records can take a few weeks. You can book appointments and use all our services straight away while records transfer.
