About this website

This site is owned and operated by Kiltearn Medical Centre, an NHS GP practice based at Church View Primary Care Centre, Off Beam Street, Nantwich, Cheshire, CW5 5NX. Our NHS ODS code is N81047. We're regulated by the Care Quality Commission and commissioned by Cheshire and Merseyside Integrated Care Board.

The site exists to give our registered patients and prospective patients the information they need to get the right care. It's not a substitute for a consultation with a doctor or nurse, and the information here doesn't constitute medical advice.

Important: Not for emergencies

If you're experiencing a medical emergency, call 999. For urgent help when we're closed, call NHS 111 or visit 111.nhs.uk. For mental health crises, call 111 and select option 2, available 24/7 and free. Don't use this website or our online forms.

Acceptable use

You're welcome to browse the site, link to it, and use the online forms for their intended purpose. You agree not to:

  • Submit false information to our online forms or impersonate someone else
  • Attempt to gain unauthorised access to the site or any related systems
  • Interfere with the operation of the site or use it for any unlawful purpose
  • Send anything that's threatening, defamatory, obscene or unlawful through our contact forms

Misuse may be reported to NHS England, the Cheshire Police, or both depending on the severity. Requests sent through our online forms are reviewed by a doctor or nurse. Frivolous or vexatious submissions take time away from clinical care.

Online forms and digital services

Online forms (consultation requests, registration, prescription requests) are not monitored in real time. We respond during our opening hours, usually the same working day. We aim to respond to:

  • Online form requests: same working day
  • Prescription requests: within 3 working days
  • Admin requests (sick notes, letters): within 48 hours

If your need is more urgent than this, please call us or use NHS 111.

Information accuracy

We do our best to keep the information on this site current and correct. Clinical guidance, NHS service availability, opening times and staff details can change. If you spot an error, please let us know.

External links go to sites we don't control. We pick reputable sources (NHS.UK, GOV.UK, established charities, peer-reviewed information) but we can't guarantee their accuracy or availability.
