All regular reviews for conditions like diabetes, high blood pressure, asthma, and heart problems are carried out by our practice nurse team.
About this service
Long-term conditions need regular review. Our practice nurses run dedicated review clinics for the main chronic diseases. We aim to see most patients with a long-term condition at least once a year.
What it covers
- Annual diabetes review including foot check and blood tests
- Asthma and COPD reviews with inhaler technique check
- Blood pressure review for patients on antihypertensive medication
- Heart failure and post-heart-attack reviews
- Mental health annual review for severe and enduring conditions
How to access this
You'll be invited by letter or text message in the month of your birthday. Phone reception to book your slot. If you haven't heard from us and think you're due, just ask.
