About this service
A long-term condition is anything that can't be cured but can be managed with medication, lifestyle changes, and regular review. Most of our patients with a long-term condition stay well for years with the right support.
What it covers
- Type 1 and Type 2 diabetes
- Asthma and COPD
- High blood pressure
- Heart disease, heart failure, and atrial fibrillation
- Chronic kidney disease
- Mental health conditions like depression and anxiety where ongoing review helps
How to access this
If you've been diagnosed with a long-term condition, we'll add you to the relevant register and invite you for regular reviews. Ask any GP or practice nurse if you're unsure whether you're on the right register.
