Keep your contact details current so we can reach you about appointments, results, and reminders.
What we need to know about
Tell us when any of these change:
- Your address, even within Nantwich.
- Your phone number, mobile or landline.
- Your email address.
- Your name, for example after marriage. Bring or attach proof such as a marriage certificate or deed poll.
Out-of-date details are the most common reason patients miss results, reminders, and appointment changes.
Update your details online
The quickest way. Fill in a short online form with your new details and our admin team will update your record, usually within one working day.
Prefer to call or come in?
Phone reception or tell us at the front desk and we’ll update your record while you wait. If you use the NHS App, you can also update some details there yourself.
Important: Moving out of our area?
If your new address is outside our catchment area, you can stay registered with us for up to 28 days while you find a new practice. Check the boundary map on our registration page.
